Included with Second Opinion Professional is this generic Referral and Consultation form. It is designed to be applicable in all medical specialties and valuable in all types of referral and consultation relationships. 

Referral Form: This form allows you to fill in a referral request and send it to remote sites. It is a generic form allowing it to be used in a variety of specialties. When consults are received in it only takes one button click to see all of the responses.

Consultation Form: When a referral form is received from a remote site one or more consultation responses are easily created and sent back.
