Cardiac Triage Form
The Cardiac Triage Form eliminates the guess work when it comes to evaluating cardiac patients. It converts a subjective referral process into an efficient, single entry referral process that places patients in a clinically determined priority for urgent cardiac care. The Cardiac Triage Form allows healthcare professionals to categorize patients by primary condition (unstable angina, valvular disease, heart failure, etc) and manage the appropriate information for that condition. It also includes other clinical information such as the patient’s cardiac history, blood work, cardiac medications, treadmill and other miscellaneous test results. Patients are then ranked and placed on a priority list in order of their relative sickness index. This insures that each and every cardiac triage examination is done the exact same way. The patients primary condition is selected to begin.
Condition Specific Data: Each primary condition has its own set of questions and each answer is assigned a weighted value to assist in the proper ranking of each patient.
Multimedia Capture: The ability to scan ECG Strips or over clinical significant reports is built right into the Cardiac Triage protocol.
Edit Records: Once the wizard driven entry process is complete each and every piece of data can be edited. This allows the system to keep up with all of the changes in the patients condition while they are awaiting transfer. Each edit is tracked as a transaction. This allows a care giver to follow trends to see if the patients condition is declining or improving.
Rank Patients: Each patient is given a transfer priority number. The primary condition weighted score assists in this process. Each different transfer area of the facility is given their own queue.
