Referral and Consultation Form

Included with Second Opinion Standard version 4.0 is a generic Referral and Consultation form. It is designed to be useful in all medical specialties for all types of referral and consultation relationships. 

Referral Form: This form allows you to fill in a referral request and send it to a remote site.

Consultation Form: When a referral form is received from a remote site, a  consultation response is easily created and sent back.
